**USA Volleyball has changed their registation to a new online registration.  Each player must pre register before try outs and pay the $25 fee.  The other $25 will be collected and paid by AZVC when you have made a team.  If you have any questions please email us at azvc@cox.net

Proof of USAV webpoint registration
Medical Release Form (click here for form) 
Birth Certificate (First year playing club ONLY)
Proof of Insurance
